Teamlet Member 3 – Clinical Associate: FTE Ratio and Qualifications

As part of the Patient Aligned Care Team (PACT), Clinical Associates are essential in supporting the care provided by Primary Care Providers (PCPs). Below are the specific staffing standards and qualifications for Clinical Associates, as well as their roles and responsibilities.

FTE Ratio Performance Standard:

  • The current standard is 1.0 FTE Clinical Associate per 1.0 FTE PCP.
  • Contractors are required to propose the mix of Clinical Associates based on the following options:

Option 1: Licensed Practical Nurse (LPN)

Qualifications:

  • The LPN must hold a current, full, active, and unrestricted license in the state where the Outpatient Site of Care (CBOC) is located.
  • If the LPN is licensed in a different state, qualifications must be in compliance with VA Handbook 5005, Appendix G13, or any subsequent policy updates.

Option 2: Licensed Vocational Nurse (LVN)

Qualifications:

  • The LVN must have a current, full, active, and unrestricted license in the state where the Outpatient Site of Care is located.
  • Similar to the LPN, if the LVN holds a license in another state, the qualifications must meet the standards outlined in VA Handbook 5005, Appendix G13 or any updated policy.

Option 3: Health Technician (Medical Assistant)

Qualifications:

  • Completion of an accredited medical assistant training program recognized by the following:
  • The Commission on Accreditation of Allied Health Education Programs (CAAHEP)
  • The Accrediting Bureau of Health Education Schools (ABHES)
  • Any accrediting agency recognized by the United States Department of Education.
  • Alternatively, candidates may be considered if they hold Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) credentials from either:
  • The American Association of Medical Assistants (AAMA)
  • American Medical Technologists (AMT).
  • Other acceptable qualifications may include completion of a military medical services training program, subject to VA Chief of Staff approval.

Option 4: Health Care Technician (HCT)

Qualifications:

  • Completion of an accredited medical assistant training program from one of the following agencies:
  • The Commission on Accreditation of Allied Health Education Programs (CAAHEP)
  • The Accrediting Bureau of Health Education Schools (ABHES)
  • Any accrediting agency recognized by the United States Department of Education.
  • Candidates may also qualify if they hold a Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) credential from the recognized professional organizations listed above.
  • Military medical training may be accepted, pending VA Chief of Staff determination.

Responsibilities for Teamlet Member 3 – Clinical Associate:

Regardless of the specific option chosen for the Clinical Associate role, the following responsibilities apply:

  • Provision of Services: Deliver covered services to both enrolled and unassigned patients presenting for care.
  • Evaluation and Care: Provide evaluations and care that align with their licensure, certification, and functional responsibilities for assigned PACT patients.
  • Collaboration with PACT: Work closely with the PACT team to create comprehensive health care and management plans for patients.
  • Clinic Workflow Management: Oversee and maintain efficient clinic workflow to ensure smooth operation.
  • Patient Movement: Ensure that patients are placed in examination rooms promptly and provide guidance to patients as they move through the clinic.
  • Specialized Clinical Support: Perform various specialized clinical support duties to assist in patient care.
  • Basic Patient Care: Carry out basic patient care services under the guidance of senior healthcare providers.
  • Medical Knowledge: Demonstrate knowledge of medical terminology and apply it accurately in practice.

  • Work closely with PACT team members to ensure that the logistical elements of care coordination are integrated into the comprehensive care management plans for each patient.

4. Guidance and Direction:

  • Provide clear guidance and support to patients and their personal support persons, helping them navigate the VA health care system and understand administrative functions.

5. Scheduling Services:

  • Provide scheduling services to patients according to the policies and guidance outlined by the VA, ensuring that appointments are made promptly and accurately.

Telehealth Clinical Technician (TCT) Staffing Standards and Responsibilities:

Staffing Requirements Based on Annual Encounters:

1. For CBOCs with fewer than 1,050 annual synchronous and/or asynchronous encounters, one TCT is required.

2. For CBOCs with 1,051-1,479 annual synchronous and/or asynchronous encounters, or those with an expansion plan target in this range, 1.5-2 TCT FTE are required.

3. For CBOCs with over 1,480 annual synchronous and/or asynchronous encounters, or those with an expansion target in this range, more than 2 TCT FTE are required.

Back-Up Requirement:

  • Each TCT must have a trained back-up (e.g., another TCT or a member of the PACT Teamlet such as a PCP, RNCM, or Clinical Associate) capable of performing all of the TCT's telehealth duties.

Qualifications:

1. Credentialing and Privileging:

  • All staff providing telehealth services (primary TCT and back-ups) must be appropriately credentialed and, where necessary, privileged.
  • All staff must work within their permitted licensure and scope of practice.
  • Non-licensed staff supporting telehealth services must be supervised by the Contractor’s licensed staff.

Clinical Supervision:

  • If non-licensed staff support telehealth services, they must receive appropriate clinical supervision from licensed personnel.

Competency and Certification:

1. Clinical Care:

  • TCTs, Telepresenters, Store and Forward Imagers, and their back-ups must provide clinical care in compliance with established clinical protocols.
  • They must complete training programs required for certification as a TCT, Telepresenter, and/or Store and Forward Imager, including VA-required training.
  • They must maintain required certification and demonstrate competency in telehealth equipment usage.

2. Equipment Competency:

  • TCTs, Telepresenters, and Store and Forward Imagers must demonstrate proficiency in using telehealth equipment (e.g., digital retinal and dermatology imaging systems, teleaudiology hardware and software).

Training:

1. Mandatory Training:

  • TCTs, RNs, LPNs, LIPs, and support staff providing telehealth services must complete all required training (virtual and in-person).
  • Staff must attend conference calls to support and optimize the delivery of care as required by the VA.

TCT Position Responsibilities:

1. Managing Telehealth Services:

  • TCT is responsible for managing telehealth services at the clinic, including presenting, equipment management, training, imaging, audiology services, etc.
  • TCT ensures the provision of covered services to both enrolled and unassigned patients presenting for care.

2. Coordination and Support:

  • Coordinating telehealth clinic setup, equipment management, maintaining records for quality control, participating in performance improvement activities, and provision of data upon request.
  • Assisting with scheduling of telehealth appointments.

3. Clinical Support:

  • TCT conveys clinically appropriate in-person interactions or on-site observations (e.g., hearing aid fittings, detection of alcohol use) with the Veteran to the telehealth provider.
  • Gathers and transmits telehealth images, sounds, data, and other supporting data to VA providers or reading centers.

4. Patient Education:

  • TCT provides technology education to patients, including:
  • Provision of VA-approved handouts.
  • Reviewing acquired data or images for anatomic and general findings.
  • Reviewing photos.

5. Communication:

  • TCT communicates regularly with the Facility Telehealth Coordinator (FTC) to address process issues, equipment needs, data collection, and other logistical issues.

Telepresenter Responsibilities:

1. Physical Exam Assistance:

  • Assist the primary care or specialty care provider with the Veteran's physical exam as needed.

2. Education and Documentation:

  • Provide Veteran education, manage required screenings, complete clinical reminders, and assist with workload capture for completing the visit, per the scope of practice.

3. Scheduling and Encounter Management:

  • Handle scheduling, opening, and closing of encounters.
